I'd be tempted to send TKI's boss (or maybe his boss's boss) a letter explaining that if this doesn't begin to be dealt with soon (say, a fortnight before TKI actually does a damn thing that doesn't involve ignoring your schedule), then you will demand a reduction in the cost of the work. When it reaches zero, you can start billing them. Don't forget to add administration charges onto it once you start sending bills. :D
